Sammendrag Forfatter: Frederike Naujokat Tittel: Sammenhengen mellom barns temperamentsstil og observert gruppekohesjon i gruppeterapi for barn med angstdiagnoser Veileder: Krister Westlye Fjermestad Bakgrunn: Angstlidelser er blant de mest utbredte psykiske lidelsene hos barn og unge. Kognitiv atferdsterapi (KAT), både i individual- og gruppeformat, er den anbefalte behandlingstilnærmingen, men allikevel opplever rundt 40% av barna ingen betydelig bedring etter gjennomført behandling. Dette understreker viktigheten av å øke kunnskapen om hva som er virksomt i KAT, både om spesifikke intervensjoner men også om generelle prosessfaktorer. Gruppekohesjon har blitt sammenlignet med alliansen i individualbehandling, men er hittil lite studert i gruppebehandling med barn og unge. Metode: Utvalget bestod av 83 barn og ungdommer diagnostisert med sosial fobi, generalisert angstlidelse og/eller separasjonsangst. Barna mottok KAT i gruppeformat ved BUP, som del av ATACA-studien (Assessment and Treatment – Anxiety in Children and Adults). Opptak av 32 gruppeterapitimer, fra både tidlig og sent i forløpet, ble skåret ved Treatment Process Observational Coding System for Child Psychotherapy – Group Cohesion Scale (TPOCS-GC). Som del av denne studien skulle også de psykometriske egenskapene til TPOCS-GC i et barneutvalg undersøkes. Barnas foresatte ble bedt om å fylle ut Behavioral Inhibition Questionnaire (BIQ), et mål for barnets temperamentsstil, med tanke på hvordan barnet var i førskolealderen. Multippel regresjon ble gjennomført for å undersøke om temperamentstrekket atferdsmessig inhibisjon predikerte nivået av gruppekohesjon. Resultater: TPOCS-GC ble funnet til å ha tilfredsstillende interrater-reliabilitet for halvparten av leddene, mens de øvrige leddene måtte utelukkes grunnet null varians eller utilstrekkelig enighet mellom koderne. Intern konsistens for reliabelt kodede ledd ble vurdert som tilstrekkelig. Regresjonsmodellene med alder og atferdsmessig inhibisjon som prediktorer predikterte både gjennomsnittlig og sen gruppekohesjon i tillegg til gjennomsnittlig endring i kohesjon fra tidlige til sene timer. Konklusjon: Resultatene fra denne studien indikerer et behov for noe revidering og muligens utvidelse av TPOCS-GC før det vil fungere optimalt i barneutvalg. Funnet at atferdsmessig inhibisjon predikerer gruppekohesjon har viktige kliniske implikasjoner både for sammensetningen, gjennomføringen og oppfølgingen av gruppeterapi med barn.
Abstract Author: Frederike Naujokat Title: The relationship between temperamental style and observed group cohesion in group therapy for children with anxiety disorders Supervisor: Krister Westlye Fjermestad Background: Anxiety disorders are among the most prevalent psychological disorders among children and youth. Cognitive behavioral therapy (CBT), both in individual and in group format, is the recommended treatment approach, however around 40% of children do not improve sufficiently after completed treatment. This emphasizes the importance of increasing the knowledge of effective factors in CBT, both of specific interventions but also of general process factors. Group cohesion has been compared to the alliance in individual therapy, but has been insufficiently examined in group treatment for children and youth. Method: The sample comprised 83 children and youth diagnosed with social phobia, general anxiety disorder and/or separation anxiety. The children received CBT in group format at community clinics, as part of the ATACA-study (Assessment and Treatment – Anxiety in Children and Adults). Records of 32 group therapy sessions, from both early and late treatment sessions, were scored using the Treatment Process Observational Coding System for Child Psychotherapy – Group Cohesion Scale (TPOCS-GC). A subgoal of this study was to assess the psychometric properties of TPOCS-GC in a child sample. The children’s parents were asked to complete the Behavioral Inhibition Questionnaire (BIQ), a measure of temperamental traits, with the child at preschool age in mind. Multiple regression analyses were conducted to assess if the temperamental trait behavioural inhibition predicted the level of group cohesion. Results: TPOCS-GC was found to have satisfactory interrater-reliability for half the items, but the remaining items had to be excluded due to zero variance or unsatisfactory agreement between raters. Internal consistency for the reliably coded items was deemed adequate. Regression models with age and behavioral inhibition as predictor variables predicted both mean and late group cohesion as well as mean change in cohesion from early to late sessions. Conclusion: The results of this study indicate a need for some revision and possibly extension of TPOCS-GC for it to work optimally in child samples. The finding that behavioral inhibition predicted group cohesion has some important clinical implications for both the composition, delivery and monitoring of group therapy with children.
