Abstract Introduction: Edmonton Obesity Staging System (EOSS) classify obese patients in stadium 0-4 based on a medical-, psychological- and functional axis. We wanted to find the proportion of metabolically healthy individuals among morbidly obese patients, find the distribution of patients based on a modified EOSS and evaluate any correlation between EOSS-stadium and achieved weight loss. Method: We graded 653 consecutive patients with morbid obesity attending the outpatient clinic for obesity at Akershus Universitetsykehus from 13.02.2013-22.12.2015 into EOSS-stadium 0-4. Patients without metabolic syndrome or weight related complications were defined as metabolically healthy. 292 patients went through interdisciplinary lifestyle treatment and achieved weight loss between different EOSS-stadiums was evaluated. Results: Of 653 patients 129 (20%) were metabolically healthy. The prevalence of EOSS 0, 1, 2, 3 and 4 were respectively 57 (9%), 81 (12%), 395 (61%), 75 (12%) and 45 (7%). Patients in EOSS 0-1 had a higher share of women (80% vs. 62%, p<0,001) and were 10 years younger (36 vs. 46 years, p<0,001) than patients in EOSS 2-4. Among the 292 patients who went through treatment there was no difference in achieved ≥5% weight loss between EOSS 0-1 and EOSS 2-4 (24% vs. 25%, p=0,810). Conclusion: More than 60% had EOSS-stadium 2. We found a lower share of metabolically healthy individuals compared to studies of patients with obesity. There were no difference between EOSS 0-1 and EOSS 2-4 and achieved weight loss. Prospective studies with longer follow up are required to assess correlation between EOSS-stadium and achieved weight loss.eng
