Ved invasiv pneumokokksykdom (IPD) er det tidligere vist i enkeltstudier at rundt 10 % av pasientene debuterer med gastrointestinale symptomer. Men, det er gjort lite undersøkelser utover dette for å karakterisere pasientgruppen. Vi utførte en kasus-kontroll-studie blant voksne pasienter innlagt med IPD på Aker universitetssykehus i perioden 1993 til og med 2008. 49 pasienter (11.4 %) hadde kun gastrointestinale debutsymptomer (GIDS) og 118 pasienter (27.6 %) hadde GIDS med eller uten tilleggssymptomer. 310 pasienter hadde ingen GIDS og utgjorde kontroll-gruppen. I forhold til pasienter uten GIDS var pasientene med kun GIDS yngre (67.2 år vs. 55.2 år), hadde færre pasienter med underliggende sykdommer (64.2 % vs. 38.8 %) og av de spesielt kronisk hjertesykdom (44.2 % vs. 16.3 %), mindre andel pneumonier (89.0 % vs. 63.3 %) og høyere andel med ukjent infeksjonsfokus (4.9 % vs. 24.5 %), samt en høyere andel pneumokokkinfeksjoner forårsaket av serotype 1 (14.7 % vs. 31.3 %). Pasienter med kun GIDS hadde en høyere 28-dagersmortalitet enn pasienter uten GIDS (28.6 % vs. 13.2 %). For pasienter som hadde GIDS med eller uten tilleggssymptomer sammenlignet med pasienter uten GIDS var funnene angående alder (59.1 år vs. 67.2 år), underliggende sykdommer (45.8 % vs. 64.2 %), kronisk hjertesykdom (21.1 % vs. 44.2 %) og infeksjoner forårsaket av serotype 1 (25.8 % vs. 14.7 %) like som hos pasienter med kun GIDS. I multivariat analyse av 28-dagersmortalitet hos pasienter med GIDS var variablene høy alder (p=0.017), lave albuminverdier (p=0.002) og høye kreatininverdier (p<0.001) assosiert med død. Cancer (p=0.049), kortikosteroidbehandling (p=0.020), kun GIDS (p=0.040), lave trombocyttverdier (p=0.007) og leukocytter i blod under 4 x109/L (p=0.003) var kun assosiert med død i univariat analyse. GIDS ved IPD forekommer hyppig og er assosiert med høyere mortalitet på tross av mindre underliggende sykdom og lavere alder enn hos andre IPD-pasienter.
Previous studies of invasive pneumococcal disease (IPD) have shown that about 10% of patients had gastrointestinal symptoms by admission to hospital. No further characteristics has been done of this specific group of patients. We performed a casus-control study among adult patients admitted to Aker University hospital and diagnosed with IPD, from 1993 through 2008. 49 patients (11.4%) had gastrointestinal symptoms at admittance (GIDS) without adjunctive symptoms, and 118 patients (27.6%) had GIDS with or without adjunctive symptoms. 310 patients had no GIDS and were designated controls. Compared to patients without GIDS, the patients with GIDS only were younger (67.2y vs 55.2y), had less patients with underlying disease (64.2% vs 38.8%) and especially chronic heart disease (44.2% vs 16.3%), less pneumonia (89.0% vs 63.3%) and more patients having an unknown focus of infection (4.9% vs 24.5%), and more infections caused by serotype 1 (14.7% vs 31.3%). Patients with GIDS only had a higher mortality at 28 days than patients without GIDS (28.6% vs 13.2%). Patients with GIDS compared to patients without GIDS had the same findings as the group with GIDS only when it came to age (59.1y vs 67.2y), underlying disease (45.8% vs 64.2%), chronic heart disease (21.1% vs 44.2%) and cases caused by serotype 1 (25.8% vs 14.7%). A multivariate analysis of patients with GIDS found the following variables associated with mortality at 28 days; high age, low serum albumin and high serum creatinine. Cancer, corticosteroid treatment, GIDS only, low trombocytes and leukocytes below 4 x109/L were only associated with mortality at 28 days in univariate analysis. P-values <0.05 were considered significant. GIDS in IPD is frequently occuring and is associated with a higher mortality despite less underlying disease og lower age than other patients with IPD.
