Abstract
Baggrund I dette studie undersøges en population på 3027 patienter med et initialt selvmordsforsøg. Det primære mål for studiet er en sub population af patienter med selvmordsforsøg og selvmordstanker (n = 377), der har deltaget i mere end en behandling over en periode på 20 år. Den valgte population er blevet sammenlignet med resten af populationen af patienter ( n = 2650 ) med kun ét behandlingsforløb. Formålet med undersøgelsen var at afklare, om det er muligt at påvise en udvikling gennem en række behandlinger, der kan bidrage med oplysninger om specifikke karakteristika for patienterne og forbedringer i deres behandling, der kunne oplyse os om effekten af behandlingen på patienterne og på at reducere deres på selvmordsforsøgs rate. Metoder Den metodiske tilgang er mixed design ( Creswell 2009) med både kvantitativ og kvalitativ metode i dataindsamlingen og i analysen. Den overliggende metodiske ramme er casestudie (Yin 2009; George 2005; Flyvbjerg 1991). Casestudiet består af en række patientforløb, hvor hver patient har haft mere end ét registreret behandlingsforløb i databasen (n = 377). Kvantitativ undersøgelse blev udført ved hjælp af Microsoft SPSS 21 på de 377 patienter på baggrund af elementer fra en klinisk spørgeskema 72 post, der er anvendt til indsamling af data om patienter siden starten på forebyggelse af selvmord i Odense i 1992. Desuden blev 56 journaler udvalgt fra populationen på grundlag af antallet af successive behandlinger og gennemgået ved hjælp af Grounded Theory tilgang ( Glaser et al 1967 Braun & Clarke 2006). Principal Components Analysis (PCA ) blev anvendt til en multivariat analyse af 32 items i datasættet. Bivariate analyser af sammenhænge mellem baggrundsvariable og de valgte emner er blevet foretaget. Resultaterne er præsenteret i frekvenser og procenter, primært i tabeller og figurer. Chi2 signifikans test er foretaget på alle analyser af forbindelser. Resultater Fra de kvalitative analyser udkrystalliseres 2 hovedtemaer, et, der indsamler oplysninger om patienterne, deres liv med sociale og psykologiske problemer og et, der indeholder oplysninger om deres relation til behandling , komplians og den mening behandlingen udgør for dem. Behandlingen synes at reducere niveauet af selvmordsforsøg gennem en forøget tendens til at søge hjælp før der forsøges igen og ved at forbedre relationelle færdigheder. De kvantitative analyser har vist signifikante forskelle mellem dem, der gentager behandling, og dem, der ikke gør det med hensyn til alder, diagnoser, misbrug af alkohol og / eller stoffer og navnlig i deres relationelle problemer. Dette støtter resultaterne af den kvalitative forskning. I den samlede population af patienter viser kronisk sygdom med smerter sig at have stor betydning og relation til stigende alder og ensomhed. Konklusion Studiet viser en signifikant sammenhæng mellem sociale og psykologiske karakteristika for patienterne og deres komplians overfor behandling og deres måde at profitere af behandlingen på. Behandlingen reducerer selvmord samt selvmordsforsøg, og terapeutiske redskaber ved behandlingen kan spores i forbedrede relationelle færdigheder mellem behandlingerne . Behandlingen i Center for forebyggelse af selvmord og dets åbne adgang til gentagen behandling synes at fungere som en "signifikant anden ", der er til rådighed, når livet bliver hårdt, der lytter uden fordømmelse og giver konkrete redskaber til at håndtere livet og relationerne. Selv over lange tidsspand husker patienterne, hvor de skal søge hjælp, og de synes at bruge anerkendelse som en måde at håndtere et liv med udbrud af selvmordstanker.
In this paper a population of 3027 patients with an initial suicide attempt is investigated. The primary target of the research presented here is a sub population of patients with suicide attempts and ideation (n = 377), who have participated in more than one treatment over a period of 20 years. The selected population has been compared with rest of the population of patients ( n = 2650) with only one treatment. The aim of the investigation was to clarify whether it was possible to detect developments through a series of treatments that could contribute with information on specific characteristics of the patients and improvements in their treatment that could inform us about the effect of the treatment on the patients and on reducing their rate of suicide attempts. Methods The methological approach is mixed design (Creswell 2009) with both quantitative and qualitative method in data collection and in the analysis. The overlying methodical framework is case study (Yin 2009, George 2005, Flyvbjerg 1991). The case study consists of a series of patient cases in which each patient have had more than one treatment registered in the database (n = 377). Quantitative study was conducted using Microsoft SPSS 21 on the 377 patients, selecting items from a 72 item clinical questionnaire that has been used for collecting data on patients since the start of suicide prevention in Odense in 1992. In addition 56 journals were selected from the population on the basis of number of successive treatments and reviewed using Grounded Theory approach (Glaser et al 1967, Braun & Clarke 2006). Principal Components Analysis (PCA) was used for a multivariate analyses of 32 items in the dataset. Bivariate analyses of connections between background variables and the selected items have been made. The results are presented in frequencies and percentages, primarily in tables and figures. Chi2 significance test have been made on all analyses of connections. Results From the qualitative analyses 2 main themes crystallize, one that gathers information on patients, their life with social and psychological problems and one that contains information on their relation to treatment, compliance and the meaning that treatment represents for them. Treatment seems to lower the level of suicide attempt by a tendency to help seeking before attempting again and by improving relational skills. The quantitative analyses have shown significant differences between those who repeat treatment and those who do not with respect to age, diagnoses, abuse of alcohol and/or drugs and in particular in their relational problems. This sustains the findings of the qualitative research. In the total population of patients chronical illness with pain shows high significance and affiliation to increasing age and loneliness. Conclusion The research demonstrates a significant connection between social and psychological characteristics of the patients and their compliance towards treatment and their way of benefitting from treatment. Treatment reduces suicides as well as suicide attempts and the therapeutic tools of the treatment can be traced in improved relational skills between treatments. The treatment in Centre for Suicide Prevention and its open admission to repeated treatment seems to work as a significant other , who is available when life gets tough, who listens uncondemning and provides concrete tools to handle life and relations. Even over long timespans the patients remember where to seek help and they seem to use the admissibility as a way of handling a life with outbreaks of suicidal ideation.
