Den kliniske fortellingen som utgangspunkt for helsefremmende dialog : En kvalitativ studie av pasienters erfaring med fortelling og samtale i psykomotorisk fysioterapi

Abstract

Oppgavens formål er å utforske pasienters erfaringer med fortelling og samtale i psykomotorisk fysioterapi, i et tilfriskningsperspektiv. Hensikten med undersøkelsen er å identifisere elementer som kan bidra til helsefremmende dialog, også utenfor en psykomotorisk kontekst. Bakgrunn for dette er praksiserfaringer og forskning som tilsier nødvendigheten av en utvidet forståelse av pasienter med sammensatte lidelser og uspesifiserte tilstander. For mange av disse er plagene vedvarende og de opplever i liten grad å bli hjulpet. En biomedisinsk sykdomsforståelse alene, synes å være utilstrekkelig i møtet med disse. Pasientenes subjektive erfaringer blir viktige kunnskapskilder i en slik sammenheng.Teori: Studien plasserer seg innenfor humanvitenskaplig sykdomslære og narrativ medisin, og tar utgangspunkt i begrepet ”den kliniske fortellingen”. Fortelling anses her som menneskelig erkjennelseskategori, og det kliniske møte betraktes som hermeneutisk og semiotisk virksomhet. Symptomer betraktes som meningsbærende tegn, og tanken er at samtaler med utgangspunkt i den kliniske fortellingen kan bidra til å avdekke ”uforklarte” symptomers budskap. Metode: Studien er forankret i en kvalitativ tradisjon og består av dybdeintervjuer av fem kvinnelige pasienter, som alle presenterer et sammensatt symptombilde. Analysen er basert på en kombinasjon av temasentrert og personsentrert meningsfortolkning. Resultater: Samtaleerfaringene er knyttet til opplevelsesmessige aspekter og betydningen av samtalen i et endringsperspektiv. Gjennom samtalene opplever informantene å bli tatt på alvor og anerkjent som unike individ, med gyldig kompetanse på egen tilstand. Analysen viser at samtalens bevisstgjørende effekt er viktig for tilfriskning; bevisstgjøring av mentale vaner og handlingsmønstre, sammenhengen mellom følelser og kroppslige reaksjoner, og sammenhengen mellom sykehistorie og livshistorie. Dette skaper grunnlag for ny forståelse for symptomers meningsinnhold. Oppsummering: Undersøkelsen har belyst pasienterfaringer med den kliniske fortellingen som utgangspunkt for samtale. Resultatene viser at den kliniske fortellingen danner grunnlag for fruktbare kliniske møter, på veien mot tilfriskning. Samtaler basert på den kliniske fortellingen bidrar til ny erkjennelse, mestring og nye fortellinger.

The objective of this assignment is to investigate patients’ experiences of narrative and conversation in psychomotor physiotherapy, from a recovery perspective. The purpose of the study is to identify elements that may contribute to a health-promoting dialogue, including outside of a psychomotor context.The background for this is the research and experiences from praxis that indicate the need for a broader understanding of patients with complex illnesses and non-specific conditions. For many of these, the afflictions are long-lasting and the degree of help they receive is small. A biomedical understanding of disease alone is regarded being inadequate in the encounter with these individuals. In such a context, the patients’ subjective experiences become important sources of knowledge. Theory: The study is situated within human scientific learning on disease and narrative medicine, and takes the concept of “the clinical tale” as its basis. The telling of tales is here considered to be a category of human recognition, and the clinical encounter is regarded as a hermeneutic and semiotic activity. Symptoms are regarded as signs that carry meaning, and the idea is that conversations based on the clinical narrative can help reveal the message of “unexplained” symptoms. Method: The study is embedded in a qualitative tradition and consists of in-depth interviews with five female patients, all presenting with a complex symptom picture. The analysis is based on a combination of theme-centred and person-centred interpretation of meaning. Results: The experiences of conversations are linked to experiential aspects and the meaning of the conversation in a change perspective. Through the conversations, the informants experience being taken seriously and recognised as unique individuals with relevant competence regarding their own condition. The analysis shows that the awareness-raising effect of the conversation is important for recovery; raising awareness of mental habits and patterns of behaviour, the association between feelings and bodily reactions, and the association between the history of illness and life history. This creates a basis for new understanding of the symptoms’ content of meaning. Summary: The study has elucidated patient experiences of the clinical tale as a basis for conversation. The results show that the clinical tale forms the basis for fruitful clinical encounters on the route to recovery. Conversations based on the clinical tale contribute to new acknowledgement, coping and new narratives.
