Abstract
Bakgrunn/Hensikt: Våkne pasienter med respiratorbehandling har reduserte muligheter til å kommunisere og selv å medvirke i omsorgen og under pleien under oppholdet. Studiets hensikt er å få kunnskap fra eksisterende praksis om hvordan sykepleierne legger til rette for at våkne respiratorpasienter gjøres medvirkende i egen pleie.
Teoretisk bakgrunn: Martinsens omsorgsteori er benyttet som forståelsesramme for medvirkning. Refleksjonsoppgaven fokuserer på deltagende observasjon, ved å benytte metodelitteratur og andre forskeres erfaringer rundt temaet; hvordan bruke seg selv som instrument.
Metode: Deltagende observasjon ble benyttet for å beskrive samhandlingen mellom sykepleieren og pasienten. Totalt åtte observasjoner ble utført.
Funn: Studiet indikerer at sykepleierne brukte et paternalistisk skjønn i samhandlingen med pasientene. Kommunikasjon med pasienter uten mulighet til å utrykke seg verbalt forutsetter at sykepleierne har tilstrekkelig tid. Samhandlingen kan preges av avdelingenes praktiske organisering og teknologien kan også bidra til distanse mellom pasient og sykepleier. Refleksjonsoppgaven viser at man som instrument og deltager i situasjonene blir involvert, og at ens eget kunnskapssyn påvirker observasjonene. Mange forskere har hatt sterke, personlige opplevelser ved bruk av metoden.
Konklusjon: Studiet viser at intensivpasientene i mange situasjoner ønsker å delta, og at sykepleierne ikke alltid har et like stort fokus på medvirkning. Refleksjonsoppgaven har bidratt til en forståelse av deltagende observasjon som en kompleks og meningsfull måte å studere samhandling på. Empiriske studier basert på refleksjon kan skape ny kunnskap og større forståelse rundt komplekse situasjoner.
Background/Purpose: Intubated, awake intensive care patients have reduced possibilities to communicate and participate in the care during the stay in the intensive care ward. The purpose of the study was to obtain knowledge from praxis about nurses facilitate for awake, intubated patients participation in their own care.
Theory: Martinsen's theory of caring as relational, practical and moral was used as an understanding for patient participation. In the essay theory is derived from methodological litterature and others experiences with participant participation, focusing on the team; How to use yourself as an instrument.
Method: Participant observation was used to describe the interaction between the nurses and the patients. A total of eight observations were performed.
Results: the findings indicate that the nurses used a paternalistic way of behaving in their interaction with the patients. Communication with patients who cannot express themselves verbally demands sufficient time. The interaction may be affected by the department's organization and technology also contributes to distance between the patient and nurse. The essay contributed to an understanding of participant observation where you use yourself as a instrument and involves in the situations. And that your personal views of knowledge influence your observations. Many scientists reflect up on their strong, personal experiences using the method.
Conclusion: The study shows that intensive care patients in many situations wish to participate and that the nurses sometimes lack a focus on patient participation. The essay has contributed to an understanding of participant observation as a complex, meaningful way of studying interaction. Empirical studies based on reflection create new knowledge that may lead to a greater understanding of complex situations.
