Abstract
Formål: Å utforske dilemmaer jordmødre erfarer vedrørende ivaretakelse av førstegangsfødende med mulighet for en normal fødsel i kvinneklinikken. Det er også et ønske om at jordmødre bevisstgjøres sitt mandat vedrørende ivaretakelse av normalfødsler. Samtidig ønsker jeg å kaste lys over særskilte utfordringer og muligheter knyttet til førstegangsfødende. Overordnet er det et ønske om at vilkårene for den normale fødsel fremmes og ivaretas i kvinneklinikken.
Teoretisk forankring: Problemstilling er utviklet nært opp til analyser av dagens norske helsepolitiske kontekst og dens framvekst. Jeg viser hvordan fødsel i dagens helsevesen hovedsakelig blir tolket og forstått ved hjelp av et biomedisinsk sykdomsparadigme. Et fenomenologisk perspektiv på helse introduseres for å få en annen og utfyllende forståelse til den rent biomedisinske. Begrepet risiko løftes også fram for å kaste lys over et sentralt aspekt ved ivaretakelse av normalfødsler i den moderne fødselsomsorgen. Jeg knytter også an til sentrale begreper hos Kari Martinsen basert på hennes omsorgsfilosofi.
Metode: Undersøkelsen er forankret i en kvalitativ metodetradisjon, og forskningsmaterialet består av dybdeintervjuer av 8 jordmødre. Min analyseform er basert på en eklektisk tilnærming med bruk av forskjellige tilnærmingsmåter og teknikker for meningsgenerering.
Resultater: I kapitlet "Tid - En tredjepart i relasjonen" demonstrerer hvordan fødselsfremgang er nært vevet sammen med hvordan relasjonen mellom jordmor og fødekvinnen får utvikle seg og samtidig hvordan denne relasjonen til tider brytes opp og fragmenteres. Det er særlig mangel på tid som utfordrer relasjonen og fødselsfremgang. I kapitlet "Tid og risiko - Normalfødselens premisser i det moderne" kommer det likeledes fram at tidsnød får markant betydning for hvilken type kunnskap jordmor benytter i fødselsprosessen. I tillegg indikerer analysen at en samfunnsskapt opplevelse av "økt personlig risiko" bidrar til en stadig mer aktiv behandling av fødselsfremgang. Avslutningsvis viser også oppgaven hvordan jordmødrenes forståelse av patologi påvirker deres syn på normalitet. Også dette aspektet synes å ha klare konsekvenser for deres ivaretakelse av fødselsfremgang.
Konklusjon: På bakgrunn av en fenomenologisk forståelseshorisont, viser oppgaven hvordan fødselsprosessen sett fra jordmødres ståsted fordrer et bredt spekter av fagkunnskap og erfaringsbasert kunnskap. Tilstrekkelig med tid er avgjørende for kunnskapsvalg og hvordan jordmor ivaretar relasjonen og fødselsfremgang.

Purpose: To explore dilemmas midwives experience from the care of nulliparous women with the possibility of expecting a normal birth at a high risk unit. There is also a wish that midwives are made aware of their mandate concerning safeguarding normal birth. At the same time I want to shed light on specific challenges and possibilities linked to the nulliparous women. Above all there is a wish that the terms for normal birth is promoted and taken care of at the high risk unit.
Theoretical perspective: The thesis is closely developed to analysis of the Norwegian health political context and its emergence. I show how labour in our health care today is interpreted and understood using a in a biomedical complementary perspective. A phenomenological perspective on health is introduced to obtain another and more complementary understanding of the biomedical. The term risk is amplified to shed light on a central aspect about doing midwifery in modern labour care. I also present central terms from Kari Martinsen based on her philosophy on care.
Method: The study is anchored in a qualitative tradition and consists of in depth interviews of 8 midwives. My analysis is based on an eclectic approach by using different approaches and techniques in generating meaning.
Results: In the chapter "Time - A Third Party in a relation" I demonstrate how the progress of labour appears to be closely interwoven with how the relation between the laboring woman and the midwife develops and at the same time how this relation at times breaks up and is fragmented. It is especially the lack of time that challenges the relation and the progress of labour. In the chapter "Time and Risk - The Premises of the Normal Birth in the Modern" it likewise appears that lack of time has a significant impact on what knowledge the midwife uses in the process of labour. In addition, the analysis indicates that a society created experience of "increased personal risk" contributes to an increasingly active management of the progress of giving birth. At the end the study shows how midwives' understanding of pathology affects their view on normality. This aspect also appears to have obvious consequences in their way of handling the progress of labour.
Conclusion: Based on a phenomenological horizon of understanding, the midwives descriptions of the process of giving birth show that it has to be met with a broad specter of knowledge, both scientific knowledge and knowledge based on experience. Sufficient time is crucial for the choice of knowledge and how the midwife takes care of the relation and progress of labour.
