Abstract
Formål: Studiens hensikt var å utvikle og pilotteste et spørreskjema som kartlegger forekomst av selvrapportert preoperativ angst hos pasienter som skal til laparoskopisk kirurgi på Dagkirurgisk avdeling, og deres begrunnelser for dette. Studien var et kvalitetsforbedrende prosjekt på Dagkirurgisk avdeling. Foruten å utvikle et spørreskjema, har metodologisk målsetting vært å vurdere hvor egnet det er til å besvare studiens problemstilling og forskningsspørsmål med tanke på målekvalitet, samt evaluere rekrutterings- og datainnsamlingsprosedyren til en senere tenkt kartleggingsundersøkelse med skjemaet.
Teoretisk forankring: Teori om følelser av R. Lazarus (1991, 1999) utgjør spørreskjemaets teoretiske forankring, og dets innhold er hentet fra relevant forskningslitteratur.
Metode: Studien har et kvantitativt, beskrivende design av type survey. En teoretisk metode for spørreskjemautvikling ble brukt for å utvikle et tredelt, strukturert spørreskjema. Spørreskjemaet ble pilottestet ved at et utvalg på 20 respondenter fra målgruppen fylte ut spørreskjemaet og deretter evaluerte det i et påfølgende strukturert intervju.
Resultater: Funn fra pilottesten viser at rekruttering og datainnsamlingen synes gjennom-førbar med tanke på oppslutning, tid, belastningsgrad for respondentene og frafall underveis. Spørreskjemaets del B og C ser ut til å måle forekomst av preoperativ angst og begrunnelser for dette. Dette understøttes blant annet ut i fra respondentenes oppfatning av spørreskjemaets elementer og utforming, spreding av skårene på anvendte Visuelle analoge skalaer (VAS) i del B, samt god indre konsistens og relativt god grad av innholdsvaliditet for spørsmålene i del C. Noen rekrutteringsfeil og feilkilder i målingene som ble påvist, bør forebygges. Behov for videre arbeid med å øke og vurdere kvaliteten i målingene ble avdekket. Blant annet bør del B operasjonaliseres bedre ved å erstatte VAS1 med instrumentet STAI (state), og spørs-målene i del C gjøres mer distinkte og vurderes videre gjennom faktoranalyse. Spørsmål om tidligere dagkirurgisk erfaring og mestringsstrategier bør legges til. Spørsmål som skapte engstelse, krever tiltak for å redusere risiko for psykisk skade hos fremtidige respondenter.
Konklusjon: Før start av en senere kartleggingsstudie, bør rekrutterings- og datainnsamlings-prosedyre og spørreskjemaet revideres i henhold til fremsatte forslag. Deretter bør spørre-skjemaet pilottestes på nytt på et større utvalg der hovedfokus er videre vurdering av validitet og reliabilitet, samt beregning av sensitivitet og spesifisitet for ulike grenseverdier på VAS.
Purpose: The aim of this study was to develop and pilot test a questionnaire that surveys prevalence of self reported preoperative anxiety with patients undergoing laparoscopic surgery in Dagkirurgisk avdeling, and the reasons patients reports for this anxiety. This study was a quality improvement project in Dagkirurgisk avdeling. Besides the development of the questionnaire an assessment of the questionnaires ability to answer this study's problem and research question with the quality of the measurements in mind and the evaluation of the recruitment and data collection methods has been goals in this study.
Literature Review: R. Lazarus' (1991, 1990) theories of emotions constitutes the questionnaires theoretical basis, and its contents is collected from relevant research literature.
Method: This study has a quantitative, descriptive design of a survey. A theoretical method for questionnaire development has been utilized to develop a questionnaire structured in three parts. The questionnaire was pilot tested by having 20 patients in the target group fill it out and afterwards evaluate it in a structured interview.
Results: Findings from the pilot test show that the recruitment and the data collection seems to be feasible with the number of willing patients, the time, the degree of stress on the participants, and the number of dropouts in mind. The questionnaires parts B and C seem to be measuring respectively prevalence of pre operative anxiety and reasons for this anxiety. This is supported by, amongst other things, the participants opinions and understanding of the questionnaires elements and their design, a good spread in the scores in the used Visual analogue scales (VAS) in part B, and a good inner consistency and a relatively god degree of content validity for the questions in part C. Some recruitment errors and measurement errors should be prevented. A need to further improve the quality of the measurements and an assessment of this is also unveiled. Amongst other things part B should be operationalized by replacing VAS1 with another instrument STAI (state) and the questions in part C should be made more distinct and should be assessed further in a factor analysis. The question about former day surgery experience and coping strategies should be operationalized into the questionnaire. Certain questions that caused unease require actions to prevent psychological harm to future respondents.
Conclusion: Before using this questionnaire in an assessment and data collection procedure, it should be developed further according to mentioned suggestions. Thereafter a new pilot test on a larger sample should be conducted. The main focus of this test should be to further assess the validity and reliability, in addition to a calculation of the sensitivity and specificity for different cut off points on the VAS.
