Abstract
Formål:
Studiens primærhensikt var å beskrive erfarne sykepleieres bidrag i en rehabiliteringsprosess. Sekundærhensikten var å finne ut hvordan taus kunnskap kan erverves i sykepleie i en rehabiliterende kontekst.
Teoretisk forståelsesramme:
Perspektiv og begreper som er knyttet til taus kunnskap og refleksjon, sykepleie, rehabiliteringsideologi, pasientens behov, rehabiliteringsprosess og rehabiliteringssykepleierens roller og funksjoner, danner det teoretiske grunnlaget for studien.
Metode:
Studien hadde et kvalitativt, hermeneutisk design. Metode for datainnsamling var halvstrukturerte intervju. Ti informanter som hadde minimum 5 års sykepleieerfaring med ryggmargsskadde pasienter ble inkludert, disse var kvinner mellom 31 og 45 år. I analysen ble det benyttet systematisk tekstkondensering.
Resultat:
Funnene indikerer at erfarne sykepleieres bidrag til den enkelte pasients rehabiliteringsprosess var av stor betydning på grunn av sykepleierens forståelse av pasienten i situasjonen og hans reaksjoner på denne. Balansering mellom realitetsorientering med krav til pasienten for måloppnåelse og å bevare håp for å unngå at pasienten ble motløs var i fokus. Sykepleiene vektla pasientens behov for selvpresentasjon og kommunikasjon.. Viktigste bidrag var å styrke pasientens vilje til rehabiliteringsarbeid. Taus kunnskap utvikles i nære stellesituasjoner med refleksjon over erfaringer, samtaler, øving og mester - svenn læring.
Konklusjon:
Erfarne sykepleiere utøver svært god sykepleie i rehabilitering på grunn av sitt oppnådde personlige kompetansenivå. Taus kunnskap er en viktig del av deres kompetanse og spiller en sentral rolle for sykepleiefunksjonen, fordi den er basert på sammenheng og helhet i en rehabiliterende kontekst, og mye i rehabilitering er avhengig av å se sammenheng.

Abstract:
Aim: The primary objective of the study was to describe hoe experienced nurses contributed to a process of rehabilitation of spinal cord injured patients. Secondly the study sought to examine how tacit knowledge is acquired and its positive influence on a rehabilitation context.
Theory: Theoretical perspectives and concepts related to tacit knowledge and reflections as well as the patients` needs during the rehabilitation process and rehabilitation nursing, represented the theoretical background of the study. In addition types of ideology of rehabilitation and multidisciplinary teamwork are reviewed.
Method: The study applied a hermeneutic approach. The method used to collect data was semi structured interviews. Ten nurses with minimum 5 years of experience from a spinal cord injury unit were sampled from 3 hospitals. The data were analyzed by use of Malteruds` systematic text condensation.
Results: Findings indicate that due to the acquired understanding of the patients` situation, the experienced nurses` contribution was crucial for the patients` rehabilitation process. The empirical work highlighted the need of balancing reality orientation with the demand for goal achievement for the patient, including sustaining hope to counteract despair and pessimism. The most important condition is to strengthen the motivation and will power of the patient to actively participate in the rehabilitation process. Development of tacit knowledge took place close to during these the caring situations involving various aspects of reflections on experience.
Conclusion: Experienced nurses` competence play an essential role in the process of rehabilitation due to their reflection which captured the totality and coherence in the rehabilitation process. Tacit knowledge accumulates by time and in perception of the patient. More research is needed for further in depth studies of health staff and patients for this theme.
