Abstract
Hensikt: Studiens hensikt er å belyse hvordan sykepleierne på en onkologisk barneavdeling erfarer møtet med foreldre til kreftsyke barn. Fokus på relasjonen mellom sykepleieren og foreldre med barn som er kreftsyke, vil kunne bidra til en utvidet kunnskap og forståelse på dette området, noe som igjen vil kunne øke sannsynligheten for en bedre hverdag for barn og foreldre på sykehus.
Bakgrunn: Det er gjort en del forskning om foreldre til barn med kreft, men den spesielle vinklingen hvor relasjonen mellom sykepleiere og foreldre til barn som er kreftsyke blir belyst, er heller lite utforsket derfor ønsket jeg å se nærmere på det. Jeg ville anvende Kari Martinsens omsorgsfilosofi for å belyse studiens teoretiske forankring. Dette formet problemstillingen og forskningsspørsmålene som er besvart i denne studien. Problemområde: Sykepleieres erfaringer med foreldre til kreftsyke barn på sykehus, med to forskningsspørsmål: Hvilke erfaringer har sykepleiere med foreldre som har kreftsyke barn på sykehus? Hvilke utfordringer erfarer sykepleiere i forhold til foreldre som har kreftsyke barn på sykehus?
Design og metode: Studiens design er basert på en beskrivende framstilling og empiriske data er samlet inn ved hjelp av kvalitative forskningsintervjuer med syv sykepleiere på en post for barn som er kreftsyke.
Resultater: Ifølge informantene er det mange forhold som skremmer barnet, både ved innleggelsen, narkosen, ved å få en diagnose, og ved behandlingen. Barnets sykdom og bivirkninger påvirker foreldrene som er i en sårbar fase, og hvordan barn, foreldre blir tatt vare på at pleiepersonalet, påvirker relasjonen dem i mellom. Av materialet fremkommer det tre faser i sykehusoppholdet: Det første oppholdet, senere opphold, og hvis barnet dør. Det er den første fasen som er mest utdypet i denne fremstillingen. Selve relasjonen mellom sykepleier og foreldre ble ikke utdypet i materialet. Videre kommer det fram i materialet at sykepleierne forteller om erfaringer og utfordringer relatert til foreldrenes sårbarhet, til betingelsene for sykepleiernes utøvelse av omsorg overfor foreldrene og til sykepleiernes ambivalens overfor foreldrene.
Konklusjon: Sykepleiernes erfaring og utfordringer knyttet til ulike aspekter i forhold til foreldre som har kreftsykebarn på sykehus ser ut til å være knyttet til foreldrenes sårbarhet fordi de er i en kritisk livssituasjon og behøver mye omsorg, informasjon og opplæring. Det ser ut som at organiseringen med primærsykepleie i en slik avdeling er av positiv art. Utfordringene ligger i hovedsak i rammer og betingelser for utøvelse av sykepleie. Det første møtet med helsesektoren, som er innleggelsen i avdelingen får stor betydning for videre forløp for alle berørte parter.
Purpose: The purpose is to illustrate how nurses in a pediatric oncology ward experience meeting with parents of children with cancer. Focus on the relationship between nurses and parents with children who have cancer, may contribute to an enhanced knowledge and understanding in this area, which in turn could increase the likelihood of a better life for children and parents at the hospital.
Background: It's done some research on parents of children with cancer, but the special angle where the relationship between nurses and parents of children with cancer are not discussed, nor is unexplored so I wanted to look into it. I would use Kari Martinsen's care philosophy to illuminate the study's theoretical framework. This shaped the approach and research questions that are answered in this study. Problem Area: Nurses' experiences with parents who have children with cancer in hospital, with two research questions: What experiences have nurses with parents who have children with cancer in hospital? What challenges experienced nurses in relation to parents who have children with cancer in hospital?
Design and Method: The study design is based on a descriptive presentation and empirical data collected through qualitative interviews with seven nurses at a hospital ward for children with cancer.
Results: According to informants, there are many factors that scare the child, both at admission, anaesthesia, by getting a diagnosis, and by the treatment. The child's illness and side effects of medication affects parents who are in a vulnerable phase, and how children and parents are taken care of by the nursing staff, affects the relationship between them. Of the material presented, there are three phases in the hospitalization: The first meeting, the next admission, and if the child dies. It is the first phase that is most elaborated in this paper. The relationship between the nurse and parents were not elaborated in the material. Furthermore, it emerged in the study that nurses tell about their experiences and challenges related to parents' vulnerability, the conditions of nurses' exercise of care to parents and to the nurses' ambivalence towards the parents.
Conclusion: Nurses' experiences and challenges related to various aspects in relation to the parents of children with cancer in hospitals appears to be related to parents' vulnerability because they are in a critical situation and need much care, information and training. It seems that the organization with primary nursing in such a hospital ward is of positive nature. The challenges lie mainly in the framework and conditions for the practice of nursing. The first meeting with the health sector, which is the admission in the ward is of great importance for further progress for all parties concerned.
